Honors Program Application Form

This form is to be filled out by the candidate to initiate the process of applying to the Honors program. A letter of recommendation, a 500-word essay and school transcripts must be attached. Incomplete forms will not be accepted.
Honors Scholarship Application Form
This form is for completion by a current Honors student to request tuition assistance. Provisional Honors students and non-honors students may not apply. Due dates appear on the form. There may be limitations on the amount of assistance as designated by the Honors Committee. A letter of recommendation and 500-word essay must be attached to the form. Completion of this form does not guarantee financial assistance. Incomplete forms will not be accepted.
Honors Community Service Approval Form
This form is to be filled out by the student and the CS supervisor before beginning community service. The student must then return the form to the Honors coordinator to be forwarded to the committee for approval. Once approved, the Honors student may start the community service work. The student may select from different types of on campus service or perform a pre-approved activity from the Honors committee. Selection of a pre-approved activity does not negate the need to first complete the community service form. There is a certification section for completion of the hours worked. Incomplete forms will not be accepted.
Honors Contract Initial Proposal Form
This form is for students to make a broad statement of what their vision of their Honors project is. The project may change from the initial proposal to the Honors contract depending on the student’s needs and/or the instructor’s requirements. Incomplete forms will not be accepted.
Honors Contract Form
This form is for the student to state in explicit detail what project the student intends to complete in order to receive an Honors designation for the class. The student and the faculty must work together to outline the project step by step to determine the due dates for all of the different parts of the project. Incomplete forms will not be accepted.
Honors Contract Final Report Form
This form is to be completed after the work for the Honors contract has been completed and a grade has been assigned. This form will state a brief summary of the project that was completed for the Honors designation. All contract work must be attached. It should be returned to the Honors coordinator as soon as it is completed so that the Honors designation may be added to the student’s transcript. Incomplete forms will not be accepted.
Letter of Recommendation Form
This form is for faculty to recommend students to the Honors Program who are capable of performing higher level work. Submission of this form does not guarantee admission to the program. Incomplete forms will not be accepted.
Graduation Form
This form is to be completed by the Honors student to apply for graduation from the Honors Program. The community service requirement must be completed prior to application. An unofficial transcript must be attached. Incomplete applications will not be accepted.
Student Progress Report
This form is to be completed by all Honors program students for every Fall and Spring semester that they are enrolled in the program. An unofficial transcript must be attached to the report. Incomplete forms will not be accepted.
