If someone other than person authorized on the registration form is to pick up your child, leave authorization in communication log, or on the site pager. This person will need a valid Photo ID.Please leave the following information:
- Name of child
- Full name of person pickup up child
- Relation of person to child
- Phone number where you can be reached
