Long Term Care and Rehab Consultants

LTC Rehab Consultants provides a bridge between a significant medical event and return to normal life. Working in partnership with primary care doctors and medical specialists, we manage the transitional care and rehabilitation process.

We work with facilities throughout the state of Colorado, including long-term care centers, skilled nursing facilities and rehabilitation centers.

As a patient-focused team of health care providers, LTC Rehab Consultants delivers personal attention and compassionate care to patients who have experienced a significant injury or health event. We simplify the process for both patients and physicians as they transition through surgery, the hospital, a rehab facility, and home care. Our goal is to ensure our patients’ successful return to daily life.

Qualifications:

  • Hospital and/or Nursing Home (long term and skilled) experience preferred;
  • Knowledge of EMR systems;
  • Computer knowledge to provide patients list, medical problem list, email, etc. . .;
  • Timely and appropriate documentation/coding;
  • Great communication skills including clear and professional phone conversation with families and other health care professional including physicians;
  • Team player with a positive outlook

We are looking for a Nurse Practitioner/Physician Assistant to diagnose and treat health conditions, as well as promote good health practices. Patience, problem-solving skills and communication ability are important. If you’re dedicated to advancing your knowledge and clinical expertise, we'd like to meet you. Immediate openings available. New graduates encouraged to apply.

Job Types: Full-time, Part-time
